Step 3: Controlled Hydrodistension with Anesthetic-Saline-Steroid Mixture

The primary distension uses a larger-bore needle connected to IV tubing to permit smooth infusion and tactile feedback for the assistant:

  • Typical injectate (commonly used in sports medicine protocols):

    • 10 mL of 1% lidocaine (fast-acting local anesthetic).

    • 10 mL of 0.5% ropivacaine or bupivacaine (longer-acting anesthetic).

    • Normal saline: Creates mechanical stretch of capsular tissue, promoting elastic and viscoelastic deformation, potentially breaking adhesions and reducing capsular stiffness.

    • Triamcinolone: Decreases synovial inflammation, downregulates cytokine signaling (e.g., IL-1, TNF-α), and reduces pain recurrence, especially useful in the painful freezing stage.


What Successful Hydrodistension Looks Like on Ultrasound

On ultrasound, successful distension is characterized by:

  • Clear needle placement under the posterior capsule with free fluid spread.
