Luteinizing Hormone (LH): This pituitary hormone signals the testes to produce testosterone. If both testosterone and LH are low, it could indicate a pituitary issue, such as a prolactinoma (a benign tumor), which warrants further investigation with a prolactin level test.
Estradiol: Men need estrogen, but the ratio of testosterone to estradiol is critical. A healthy ratio is about 20:1. As testosterone drops, estrogen can rise, leading to symptoms like gynecomastia (breast development).
Sex Hormone-Binding Globulin (SHBG) and Free Testosterone: This is especially important for obese men. Obesity often lowers SHBG. Since SHBG binds to testosterone, a lower level can make the total testosterone appear normal while the biologically active free testosterone is actually low.

Over 50% of obese men exhibit low testosterone. This connection is a two-way street: low testosterone can promote fat storage, and excess body fat can lower testosterone. Furthermore, low T is associated with an elevated risk for:
Type 2 Diabetes: Higher testosterone levels are linked to a lower risk.
Hypertension and Hyperlipidemia.
Sleep Apnea: It's vital to screen for sleep apnea before starting testosterone replacement therapy (TRT), as TRT can relax neck musculature and worsen the condition.
Rheumatoid Arthritis.
Testosterone Replacement Therapy (TRT): Goals and Guidelines
When TRT is indicated, the primary goal is not to chase a specific number but to alleviate symptoms. The therapeutic target range is typically between 450 and 600 ng/dL. While TRT is not a primary treatment for ED, it significantly improves libido and can enhance the effectiveness of other ED treatments like PDE5 inhibitors, often counteracting the tachyphylaxis (diminishing response) that develops over time.
Before starting TRT, it's essential to conduct a full history and physical to rule out other causes for the symptoms. Key considerations include:
Prostate Health: We monitor PSA levels and perform digital rectal exams. The old fear that TRT causes prostate cancer has been largely debunked. Current guidelines allow for TRT in men who are 1-2 years post-treatment for prostate cancer, provided they are closely monitored.
Cardiovascular Health: TRT should be avoided within 3-6 months of a major cardiac event, and consultation with a cardiologist is recommended.
Hematocrit Levels: TRT can increase red blood cell production (erythrocytosis), which raises the risk of blood clots. We monitor blood counts regularly and adjust the dose or temporarily pause therapy if the hematocrit rises too high.
